We will learn about your needs, expectations and requirements and a Care Coordinator will be assigned to you.
Through consultation and careful planning, a Care Plan will be prepared for you, taking into consideration your health, background, preferences, and any other important information pertaining to your health, well-being and independence.
How do you match care staff and clients?
Your Care Coordinator will be your first point of contact for any questions or concerns and will guide you through the process. We take into account your interests, culture, home language and preferences to match you to the right staff member who will be your carer for the duration of your service.
What happens if I want to change my services around?
Your Care Coordinator is the single point of contact for all your services. If you want to make changes to your services and it’s within the scope of your package, your Care Coordinator can do this.
What happens if I want to change my carer?
Being comfortable with your carer is very important and that’s why we spend time in the initial stages learning as much as we can about you and your family. However, if for whatever reason you are unhappy with any staff member sent to your home, your Care Coordinator will be the first point of contact. We have many different care staff working for us, so we are certain we can find you a carer you can trust and feel comfortable with.
How often do you review services?
Your Care Coordinator will be checking in with you regularly, and your care plan will be reviewed every six months. However, your Care Coordinator is always available for any questions you may have.
Do you have any hidden fees or exist charges?
There are no hidden costs or exit fees. All of our pricing is fully transparent and can be viewed here.
What happens if I want to leave 365 Care?
If, for whatever reason, you decide to terminate your service, you can. We have no lock-in contracts or hidden charges.
